Description:
Project REACH is a residential facility designed to reach the most intractable of the homeless population; adults with severe and persistent mental illness as well as chronic substance abuse/dependence. These individuals are commonly referred to as the dually diagnosed.
The facility can house up to nine adults. The specific population targeted are homeless men and women who suffer from a psychotic disorder primarily and substance abuse secondarily. Residents may stay in the program for up to twenty-four months during which time room and board is provided for a rental amount based on the resident's income. Residents are expected to participate in a variety of therapeutic activities and interventions designed especially for individuals with dual diagnoses.
An eligible person is defined as an individual who:
*Is an adult(18years or older);
*Is diagnosed with a DSM-IV Axis I psychotic disorder (e.g. schizophrenia, bipolar disorder, major depression) with a documented history of mental health treatment;
*Is diagnosed with a DSMIV Axis I substance use disorder (i.e. abuse or dependency) with a documented history of substance use;
*Is psychiatrically stable (i.e. client is not in need of inpatient care, is on medication to control symptoms of mental illness, and does not have a medical condition requiring the care or supervision of medical personnel);
*Is homeless or at risk of becoming homeless; and
*Is able to make his/her own choice about whether or not to enter the program.
Project REACH provides the following range of services:
*Residential Services-includes shelter, meals, twenty-four hour supervision by paraprofessional staff and law enforcement officers.
*Mental Health Services-includes regulation of appointments with treating psychiatrist, medication monitoring, counseling and therapy, twenty-four hour crisis intervention.
*Substance Abuse Treatment-includes Relapse Prevention/Recovery Planning, AA/NA meetings, individual, group and family counseling, random drug testing
*Case Management Services-includes assessments, linking residents with entitlements, development of board and care/service plan, coordination and monitoring of mental health services, transportation.
*Psychosocial Skills Training (daily living skills)-includes housekeeping skills, personal care and hygiene, money management, use of public transportation, work adaptation skills, symptom management skills, social and recreational skills, coping skills, etc.
*Law Enforcement Support-includes protection of residents and staff, management of violent and aggressive behavior, crisis intervention, provision of positive interaction with law enforcement officials
*Follow Along-includes community integration, supported living services, support groups
There are four important goals that each residents is expected to work toward:
(1)maintenance of mental health stability;
(2)abstinence from drugs and alcohol;
(3)permanent housing; and
(4)attainment of food stamps.
In addition to these programmatic goals, residents are assisted with setting and reaching individualized goals. When a resident has satisfactorily reached these goals according to his/her individual capabilities then the resident may graduate from the program.
