Intake Process:

Please bring all available information, including:

*Photo ID (Required)
*Social Security card or Individual Taxpayer Identification Number (ITIN)
card or letter for your self and each person to be claimed on your tax return (Required)
*Birth dates for every person to be claimed on your tax return (Required)
*W2 or 1099 forms for all jobs held (Required)
*End of the year statements for other income you received (including bank interest, social security income, unemployment, gambling, etc.) (Required)
*End of the year retirement plan statements (401 (k), IRA, etc.) (Required)
*Child care provider name, address and tax ID number
*Record of expenses (child care, education, medical, dental, etc.)
*Homeowners Insurance Citizens Declaration
*For a faster refund, a blank check or deposit slip, with your account number and bank routing number

U. S. Citizen or Legal Permanent resident
Program targets low-moderate income families whose AGI is less than $52,427, but will not turn away based on income
