Satellite clinic of Iberia Comprehensive Health Care Clinic.

COMPREHENSIVE MEDICAL SERVICES:
--Care for acute illness
--Care for chronic conditions
--Care for minor emergencies
--Cardiac (heart) problems
--Children and adult immunizations
--Health Promotion counseling
--Men's health program
--Physical examinations: sports, employment, school, etc.
--Preventive medicine
--Referrals for specialty care
--Wellness program
--Women's health program
--Flu/Pneumonia Vaccines
--Physicians linked to Dauterive and Iberia Medical Center for hospitalization referrals
PHARMACY/MEDICATION SERVICES:
Full time staff works with various pharmaceutical companies to obtain either free or discounted prescriptions for qualifying patients based on income and family size.
Available to patients who:
--Do not have a prescription reimbursement plan
--Do not qualify for local, state, or federal prescription reimbursement
--Fall below Federal Poverty Guidelines
MENTAL HEALTH SERVICES:
--Psychiatrists and Licensed Clinical Social Workers on staff
--Children, adolescent and adult mental health care and counseling
LABORATORY SERVICES:
--Reduced rates on laboratory testing
--Uninsured patients receive additional discounts based on income and family size
IMMUNIZATIONS:
--Adult and children immunizations
--Flu & Pneumonia vaccines available during the season
-Immunizations provided four days per week: Monday, Tuesday, Thursday and Friday. Cost same as that paid at the Public Health Units
COMPREHENSIVE DENTAL SERVICES:
Preventative and restorative care for patients three (3) years of age and over:
--Cleaning and Oral hygiene
--Fluoride Application
--X-rays
--Extractions and other minor surgery
--Dentures (Full, partial and refining)
--Periodontics (Scaling and root planning, etc.)
--Endodontics (Root canals, etc.
--Same day emergency service for patients in pain (slots are limited and patients must call center for instructions before arriving for same day emergency treatment)
-Discounted rates for individuals who do not have insurance (based on income and family size)
Because fees are already discounted, dentures, partials, crowns and other specialty services are not eligible for sliding fee discounts.

M, 8:00 a.m. - 5:00 p.m. ;
Monday (Extended Hours) 5:00 p.m. - 7:00 p.m.;
TU, TH 8:00 a.m. - 5:00 p.m.;
W, 8:00 a.m. - 2:00 p.m.;
F, 8:00 a.m. - 5:00 p.m.;
Fourth Friday of every month, closes at NOON.

Intake Process:

Call before going. Call for an appointment. Proof of coverage for Medicare, Medicaid and Private Insurers: Proof of Income & expenses, proof of residency, tax returns, check stubs

Sliding Scale for Self Pay patients; Free for indigent; accepts Medicaid, Medicare, most private insurance carriers
